Why You Should Never Fixate on a Single Biomarker

Why does a single value say so little?

Because lab values fluctuate — for two reasons. First, biologically: your body isn't a machine. Time of day, your last meal, hydration, exercise, stress, sleep, even a brewing cold shift many values. Second, technically: every measurement carries a small degree of uncertainty.

On top of that comes a misunderstanding about the reference range. It isn't the border between "healthy" and "ill"; it describes the range in which the central 95% of a healthy comparison group fall. The statistics trap: why "one abnormal value" is often normal

This is where it gets interesting. If a single test flags 1 in 20 healthy people as "abnormal," what happens when you measure many values at once?

The probabilities add up. With a panel of 20 markers, the probability that not a single one is flagged by chance is only 0.95²⁰ ≈ 36%. Put differently: around 64% of perfectly healthy people have at least one value outside the reference range across 20 tests — purely by chance [1].

That isn't a weakness of diagnostics; it's simple statistics. And it's why you can't build a diagnosis from one red value in a large panel. The skill isn't finding every deviation — it's recognising which one is meaningful, and which is noise.

Risk is almost never a single number

Take cholesterol. Many people fixate on the LDL value alone. But actual cardiovascular risk arises from several factors together. The European guidelines (ESC) therefore calculate it with the risk model SCORE2 — from age, smoking status, blood pressure and total plus HDL cholesterol combined, not from a single value [2].

The same principle applies everywhere: a single blood-sugar value says less than long-term blood sugar plus insulin. A single blood-pressure reading says less than several readings over time. Health is a web of values, not a single node.

Snapshot or trend — which is more telling?

Almost always the trend. A single value is a snapshot; only the development over time shows a real pattern. A value that has been stably slightly elevated for years means something different from one that has climbed sharply in six months — even if both look identical today.

So is measuring a lot pointless?

On the contrary — but only with interpretation. This is exactly where the misunderstanding lies: the problem isn’t broad measuring, it’s interpreting single values in isolation, without context.
