Do I have to take this forever — and what happens if I stop?” It’s a fair question, and it deserves a clear answer rather than a marketing promise.\n\nThis article explains what happens when you stop, why it happens — and above all how to hold on to what you’ve achieved, if and when you reduce the treatment.\n\n## **What happens when you stop a weight-loss injection?**\n\nTwo things, fairly reliably: your **appetite returns**, usually within a few weeks. And with it, part of the weight often comes back.\n\nHow much? In the extension of the STEP-1 trial, participants regained on average about **two-thirds** of their lost weight one year after stopping — but on average they were still around **5.6% below** their starting weight (Wilding et al., 2022) \\[1\\]. The good news is that regaining weight after stopping treatment does not automatically mean that all the progress you've made is lost.\n\n## **Why does the weight come back? Is it my fault?**\n\nNo. Regain is **not a sign of weakness or a lack of discipline**. Obesity is a recognised **chronic disease**. **GLP-1 medications** treat obesity for as long as they are used, but **they do not cure the disease**. After treatment is discontinued, hunger, **\"food noise,\"** and the body's biological counter-regulatory mechanisms may return or become more pronounced.\n\nA helpful analogy is blood pressure treatment: its benefits generally last only as long as the medication is taken. This is a normal response of the body, not a personal flaw.\n\nBut “forever” isn’t a given. Whether, when and how you reduce the dose or pause is an **individual, medical decision** — depending on your goal, your health and how stable your result is. What matters is less the “whether” and more the “how”: planned rather than abrupt.\n\n## **How do you keep your result?**\n\nThis is where the real leverage lies — and it’s greater than many people think.\n\n**Don’t stop abruptly; taper.** Reducing the treatment slowly rather than stopping overnight gives appetite and metabolism time to adjust — best supported by nutrition and exercise. That helps limit regain.\n\n**Muscle and movement protect your result.** In one analysis, physical activity markedly improved the odds of keeping the weight off. **The reason is simple:** muscle helps support energy expenditure and reduces the metabolic adaptations that occur during weight loss. That’s why it’s worth looking at body composition already **during** treatment (more on this in our article on muscle and GLP-1). Rule of thumb: enough protein and strength training two to three times a week.\n\n**A lifestyle that stays.** The habits you build during treatment—appropriate portion sizes, adequate protein intake, regular physical activity and good sleep—lay the foundation for what comes afterwards.
